Passports

• Please note that your appointment is not confirmed until you receive the second confirmation email (not the first pending email).

• Minors must be accompanied by both parents/guardians. If both parents/guardians are not present for the appointment, the library cannot accept the application unless parents/guardians submit the Statement of Consent for Issuing a Passport to a Child (DS-3053) or the Statement of Special Family Circumstances (DS-5525).

Please remember to bring with you:

• Completed application form

• Your personal check or money order

• Photo id 

• Original copy of proof of citizenship (i.e. birth certificate) 

• Passport photo.

Note: Because each appointment is for one hour, the last appointment that can be scheduled is at 3 pm. The physical space reverts to general use at 4 pm.
